The rules every matrix states

Each task needs exactly one person accountable: the one whose call it is and who answers for it. Two accountable people means each can assume the other has it; none means nobody does. Each task needs at least one person responsible for doing the work. The tool checks every row for both, and questions rather than fails a task with an accountable person and no one responsible, because in many teams the accountable person does the work too, and the fix is to write A/R.

Too many voices

Four or more people responsible for one task, or most of the matrix consulted on it, is flagged. Each consulted person is a conversation that has to happen before the work moves, and a task everyone is responsible for tends to be done by no one in particular.

The view down the columns

A row-by-row check misses what the columns show. The tool counts, for each person, the tasks they are responsible or accountable for, and flags anyone carrying at least twice the median, and anyone accountable for more than half the tasks, where every decision waits on one person. People who are only ever informed are listed too: being told is a real role, but it may belong on a distribution list rather than a column.

Import or build

Import a matrix with tasks down the side and people or roles across the top, or a list with one row per assignment. Letters can be in capitals or not and combined in one cell (A/R, R, A). Or build one on the page, starting from an example, and download it with the findings.

What this does not do

It checks the structure of the matrix, not whether the right person holds each letter; that depends on the people and the work. It reads R, A, S, C and I; other schemes (DACI, RAPID) use different letters and different rules.
